Country star Mindy McCready is the fifth former contestant on Celebrity Rehab reality show who died within three years of appearing on the television show.

Since Mindy McCready, 37, was found after an apparent suicide at her cabin in Arkansas, Celebrity Rehab has come under increased scrutiny.

However, Mindy McCready herself was possibly under scrutiny at the time of her death. Police were reportedly planning question her about the death last month of her boyfriend David Wilson, who was shot at the same Heber Spring, Arkansas, home where Mindy McCready killed herself.

Celebrity Rehab, which is currently in its fifth season on VH1, is lead by Dr. Drew Pinsky and features a changing cast of celebrities. Camera chronicle their journey through a 21-day rehabilitation clinic where they are treated for various drug addictions.

In addition to Mindy McCready, four other castmates have died because of different incidents relating to their addictions.

The most fatal of the show’s seasons was the third season, since Mindy McCready’s death brings the fatality count up to three out of the nine featured cast members.

It was revealed on Monday that Mindy McCready had been working on a suicide awareness video right before her death, it was revealed today.

Private investigator Danno Hanks told E! News Mindy McCready had asked him to work on a cover of I’ll See You Yesterday which was written by Courtney Dashe, a fan of McCready’s who wanted her to sing the song.

Then on Saturday, the day before she was found dead from a self-inflicted gunshot wound at her Arkansas home, Mindy McCready sent him a frame for a video with a suicide hotline number.

He said: “She said she wanted to put together this video to help people deal when someone had suffered a suicide in their life and God, she was literally having me write her suicide note.

“If I had known, I would have made sure someone was there with her all the time. I thought she was talking about sending a message about suicide for David’s death but this is unbelievable.

“She was sending me a message. I wish I had been more alert to what it was. I just didn’t think she would do this. She had two kids to live for. I still can’t fathom the whole thing.”

Mindy McCready was also facing public scrutiny over the death of David Wilson on January 13.

Sheriff’s deputies investigating the case had said the death was suspicious. They were planning to re-interview before she died, People magazine reports.

Mindy McCready denied having anything to do with his death – calling David Wilson her “soulmate” – but police said unanswered questions remain.

“At first, she said she hadn’t heard the gunshot because the TV was too loud. Then she said she had heard the gunshot,” a source told People.

“So obviously there were a lot of questions, and the Sheriff was asking for clarification.”

Former Real World contestant Joey Kovar died in August of last year, just three years after the third season of Celebrity Rehab aired.

Though his family said that he had maintained his sobriety following the show, an autopsy revealed that his cause of death was opiate intoxication.

That is believed to be a result of the painkiller that Joey Kovar was taking at the time, which was called Norco and classified as an opiate though it was never confirmed that the drug caused his death.

Their fellow cast mate Mike Starr, a bass player from the grunge rock band Alice in Chains, was arrested in February 2011 for possessing two different types of prescription medication that were deemed controlled substances.

Mike Starr was found dead less than a month later as a result of what Dr. Drew deemed a prescription drug overdose.

The two other stars who died after their series aired were actor Jeff Conaway of Grease fame and Rodney King, the man whose beating sparked the Los Angeles police brutality riots.

Jeff Conaway, who made a name for himself as Kenickie in the musical film, was featured on both the show’s first and second seasons because his addiction to prescription medication returned following back surgeries.

At first it was thought that he died in May 2011 due to a drug overdose, but Dr. Drew later made a public statement saying that he was placed in a medically induced coma following a combination of pneumonia and sepsis caused by years of drug abuse.

Rodney King, who shot to fame after he was seen being beaten by police in Los Angeles which went on to prompt the infamous riots of the early 1990s, was found dead after having drowned in his swimming pool.

The drowning was ruled an accident but an autopsy proved that he had a mix of cocaine, marijuana, and alcohol in his system at the time of his death in June of last year.

Now much of the criticism is falling on the shoulders of Dr. Drew Pinsky who serves as a host, executive producer and doctor on the show.

“Although I have not treated her for a few years, I had reached out to her recently upon hearing about the apparent suicide of her boyfriend and father of her younger children. She was devastated,” Dr. Drew Pinsky said in a statement Monday.

“Although she was fearful of stigma and ridicule she agreed with me that she needed to make her health and safety a priority. Unfortunately it seems that Mindy did not sustain her treatment.”

Mindy McCready had been in the news over the past month because her live-in boyfriend David Wilson also died in an apparent suicide at the same cabin in Heber Springs.

Mindy McCready was allegedly in the house at the time of his death but she allegedly did not hear the gun shot because the television was too loud.

Following David Wilson’s death, she attended court-ordered rehab and her two sons were put into foster care, and they remained in the custody of the state at the time of her death.

On her bio page from Celebrity Rehab’s website, VH1 made references to her personal problems and even pegged the root of her troubles on her relationships.

Mindy McCready’s biography on the show’s page says: “Although she was arrested and jailed for trying to buy OxyContin with a false prescription and attempted suicide with an overdose of drugs and alcohol, Mindy believes her only true addiction is to violent relationships.”

Mindy McCready, 37, died from an “apparent self-inflicted gunshot wound”, Arkansas sheriff Marty Moss said.

The troubled singer had attempted to take her own life twice before and was known to have problems with prescription drugs and alcohol.

Mindy McCready’s death comes weeks after that of her boyfriend and father of one of her two sons, record producer David Wilson, whose death is being investigated.

The singer recorded five studio albums.

NBC reporter Andrea Canning tweeted: “Just got a call from Mindy McCready’s best friend that she shot and killed herself this evening.”

Police said they had found the body of Malinda Gayle McCready on the front porch of her home in Heber Springs. She was pronounced dead at the scene.

After the death of her partner in January, Mindy McCready had told NBC’s Dateline programme that she had “never gone through anything this painful.

“He didn’t just touch my heart, he touched my soul.”

Mindy McCready denied any involvement in David Wilson’s death.

Her two boys – a six-year-old and a nine-month-old – were removed from her home earlier this month when she was ordered to undergo rehabilitation.

She was discharged last week for outpatient treatment, but her sons remained in foster care.

Mindy McCready had been arrested several times on drug charges. She also made headlines for claiming to have had a 10-year affair with former baseball star Roger Clemens that began when she was a teenager.

Her best-selling album was her 1996 debut Ten Thousand Angels which included the hit Guys Do It All The Time. Her fifth album, I’m Still Here, was released in 2010.

David Wilson, the live-in boyfriend of troubled country singer Mindy McCready, shot himself in his Arkansas home on Sunday night.

New sheriff reports reveal that David Wilson, 34, was found in his house in Heber Springs with a self-inflicted gunshot wound and he was transported to the local hospital but declared dead shortly after.

Mindy McCready, who has publicly struggled with drug problems, described her live-in boyfriend as her “soul mate” and “life partner” as she announced his death on Sunday.

The official Cleburne County Sheriff’s department confirmed that police and emergency services responded to a call at approximately 6:15 p.m. on Sunday though it is unclear who made the call.

The scene is a familiar one to police, as David Wilson was found shot in the same house where Mindy McCready was discovered hiding in a closet in December 2011 with her elder son Zander after a nationwide manhunt as part of a long custody dispute.

Zander was taken from Mindy McCready’s arms and placed in foster care.

Mindy McCready’s mother and step-father had legal custody of him at the time, following McCready’s lengthy and public battle with drug addiction.

It’s unclear whether Zander is living with Mindy McCready, if he has been returned to his grandparents and if he remains in foster care.

David Wilson is the father of Mindy McCready’s younger son, Zayne, who was born in April.

“Today, we mourn the loss of my life partner and my children’s father, David Wilson,” Mindy McCready said in a statement she released.

“David was my soulmate; he was a caregiver and guiding hand to our sons, Zander and Zayne.

“He was a precious gift from God to all of us and, yesterday, he returned home and is now with his mother and father. David loved and he was loved.

“Those who knew and loved him will miss him; those who did not know David missed the opportunity to know a truly loving and gifted man.”

The police report says that emergency services arrived at David Wilson’s house on Fox Chase Road at 6:15 p.m. and transported him to nearby Baptist Hospital, where he was pronounced dead at 8:00 p.m.

No criminal charges have been filed but the investigation is ongoing.

Mindy McCready’s father Timothy said that the family were still coming to terms with their loss.

The house on Fox Chase Road became the center of the custody battle between Mindy and her parents after she kidnapped her elder son and hid out in the house that was then-owned by her father Timothy McCready. News reports said at the time that neither the singer nor David Wilson- who was found in the house with her- were allowed to be on the property.

The house has since legally changed hands, and is now owned by a relative of David Wilson’s.

David Wilson is said to be a record producer and songwriter, though it is unclear who he worked with specifically.

Mindy McCready rose to fame in 1996 with her hit country song Ten Thousand Angels. Her album of the same name went double platinum – though that marked the peak of her career.

She struggled with arrests and drug addiction after her career began to faded in the late 1990s.

She was dropped by BNA Records in after her 1999 album had disappointing sales and then dropped by Capitol Records in after her 2002 record suffered the same fate.

Mindy McCready was arrested in Tennessee in 2004 and pleaded guilty to using a fake prescription to buy the painkiller OxyContin.
She was arrested in Nashville in May 2005 for drunk driving and driving with a suspended license after she was stopped for speeding. Two months later, Mindy McCready was charged with identity theft, unlawful use of transportation, unlawful imprisonment, and hindering prosecution in Arizona.

She was arrested twice, in 2005, 2007 and 2008, for violating the term of her probation.

In July 2007, Mindy McCready was arrested in Fort Myers, Florida and charged with fighting with her mother.

In 2005 and 2008, Mindy McCready was hospitalized for attempting suicide. She was taken to the hospital in 2010 for a drug overdose, as well.

Mindy McCready has been mounting a comeback in recent years, though. Her 2010 album I’m Still Here was praised by critics for her vocal prowess.

She also appeared on Celebrity Rehab with Dr. Drew as she attempted to work out her drug issues.

Mindy McCready was found hiding in a closet with her five-year-old son last night after defying a court order to return him to legal guardian.

Police raided the Arkansas home of Mindy McCready’s boyfriend at around 8:00 p.m. after tracking the pair down.

The country singer has been accused of stealing the boy from her mother, Gayle Inge, who has had custody of the grandson since 2007, during a recent visit to her Florida home.

A judge had signed an order for Mindy McCready’s son, Zander, to be taken into custody if he was not returned to his legal guardian by 5:00 p.m. on Thursday.

Florida Department of Children and Families spokeswoman Terri Durdaller said that her agency was working with Arkansas state officials to bring the youngster back to his maternal grandmother in Florida.

Officials say the boy is safe and in good health.

Gayle Inge was tearful when she spoke about the news last night.

“I’m real excited that he’s safe,” Gayle Inge said.

“But I can’t explain what this is like. We feel for Mindy and we feel for Zander.”

Gayle Inge said that her son – Mindy McCready’s half-brother – texted his sister, who responded with a text which said her mother would never see her again.

“I want to wrap my arms around her and tell her that I love her,” Gayle Inge said, adding that her daughter and grandson were found by authorities “hiding in a closet”.

The evening’s developments capped a days-long struggle between Mindy McCready – who is seven months pregnant with twins – and child welfare authorities and her own mother.

Authorities say Mindy McCready – who turned 36 on Wednesday – took the boy during a visit late last month to her father’s Florida home, where she was allowed to visit the boy.

Mindy McCready’s parents are divorced.

A Florida judge signed an order on Thursday telling authorities to take the boy into custody and return him.

It’s not yet clear whether the country singer could face criminal charges.

Mindy McCready said earlier in the week that she would not bring her son back from Tennessee, where she has a home, despite violating the custody arrangement.

The singer said that her son had suffered abuse at her mother’s house, a claim that Gayle Inge vehemently denies.

“I’m doing all this to protect Zander, not stay out of trouble,” Mindy McCready wrote in an email to The Associated Press (AP) on Thursday.

“I don’t think I should be in trouble for protecting my son in the first place.”

Mindy McCready told the AP on Wednesday night she was in Tennessee and could not travel because she is pregnant.

The boy’s father, Billy McKnight, told NBC’s Today show on Friday that he spoke on the phone with Mindy McCready and their son after the judge’s 5:00 p.m. Thursday deadline expired.

“He did sound healthy and ok. He wasn’t crying or scared,” Billy McKnight said about their son.

“I think she believes she has a case and doesn’t realize she’s pushing her luck on this one.”

Mindy McCready and her mother have had a long custody battle over the boy.

The singer had provided a series of emails to the AP with Lee County Judge James Seals’ ruling to return the boy.

“Mom has violated the court’s custody order and we are simply restoring the child back into our custody,” the judge wrote.

“Nothing more. Nothing less. The court makes no judgment about whether Mom will or will not competently care for the child while in her custody.

“It only wants the child back where the court placed him.”

Mindy McCready found fame in the mid-1990s when she moved to Nashville at the age of 18, armed with only her karaoke tapes.

Her first album, “Ten Thousand Angels”, sold two million copies.

Mindy McCready’s next four albums weren’t as successful. Her personal troubles began encroaching on her professional success.

According to her website, Mindy McCready suffers from severe depression.

In August, the singer filed a libel suit against her mother and the National Enquirer’s parent company, American Media Inc., over a story published in the tabloid newspaper that quoted Gayle Inge.

In 2008, Mindy McCready was admitted to a hospital after police said she cut her wrists and took several pills in a suicide attempt.
